How to Make Your claim

 

Please select the type of claim/s being made and provide the information and documents required to enable us to proceed with the handling of your claim. Please contact us in case of need.

 
 
 

Cancellation & Abandonment

 
  • Completed Claim Form

  • Passport Copy

  • Copy of ID card of Main Card Holder

  • Flight Tickets or e-tickets (departure and arrival)

  • Hotel Accommodation Vouchers

  • Flight & Accommodation invoices & Receipts;

  • Tour / Holiday Programme;

  • Documentation to confirm reason/s for cancellation or curtailment:

  • Medical Report/s;

  • Death Certificates.

  • Cancellation confirmation from Airline / Agent indicating any refund due (if any)

 
 
 

Medical  Emergence & Associated  Expenses

 

  • Completed Claim Form

  • Passport Copy

  • Copy of ID card of Main Card Holder

  • Flight Tickets or e-tickets (departure and arrival)

  • Medical Reports

  • Medical Receipts

  • Receipts for additional accommodation and travelling

  • The member may be examined by a consultant appointed by MIB

 

 

Hospital Benefit

  • Hospital Reports

Personal Liability

 
  • Completed Claim Form

  • Other documentation to substantiate the Member’s claim depending on type of claim

  • Members are to ensure that similar claims are to be handled by

  • MIB and no commitment is made regarding settlement or payment

 
 
 

Delayed Departure

 
  • Completed Claim Form

  • Flight Tickets or e-tickets (departure and arrival)

  • Written declaration from Airline confirming the reason and hours of delay

 
 

Missed Departure

 
  • Completed Claim Form

  • Passport Copy

  • Copy of ID card of Main Card Holder

  • Flight Tickets or e-tickets (departure and arrival)

  • Reason for missed departure substantiated by documentation

 
 

Personal Accident

 
  • Completed Claim Form

  • Passport Copy

  • Copy of ID card of Main Card Holder

  • Medical Reports

  • The member may be examined by a consultant appointed by MIB

 
 

Baggage / Personal Belongings

 

  • Completed Claim Form

  • Passport Copy

  • Copy of ID card of Main Card Holder

  • Flight Tickets or e-tickets (departure and arrival)

 

Damaged Suitcase:

  • Damage report by Airline / Cruise liner (Property Irregularity Report);

  • If no report is available the Member is to lodge a report in writing with the respective airline within 3 days from arrival

  • Original Baggage Tags;

  • Boarding Passes;

  • Details of damaged suitcase (brand/colour/size/type/damages/measurements)

  • Damaged suitcase is not to be disposed.

 

Non-delivery of Suitcase:

  • Property Irregularity Report issued by the Airline / Carrier;

  • Original Baggage Tags;

  • List of Missing Items;

 

Stolen / Lost Items:

  • Police report within 24 hours of discovery

  • Receipts / Valuations / Proof of Ownership of lost / stolen items (if available)

 

 

Delayed Luggage:

 
  • Completed Claim Form

  • Passport Copy

  • Copy of ID card of Main Card Holder

  • Flight Tickets or e-tickets (departure and arrival)

  • Property Irregularity Report issued by the Airline / Carrier;

  • Written declaration confirming the date and time when the luggage was collected;

  • Baggage Tags

  • Receipts for emergency expenses incurred.

 
 

Loss / Theft of Personal Money:

 
  • Completed Claim Form

  • Passport Copy

  • Copy of ID card of Main Card Holder

  • Flight Tickets or e-tickets (departure and arrival)

  • Evidence of the stolen wallet;

  • If any cards were stolen, then we require evidence that the same cards have been stop listed with the bank/s;

  • Evidence of the money taken abroad. This may take the form of foreign exchange receipts or cash withdrawals in the case of Euros;

  • Report from the relevant authorities, for example Police

 
 

Loss of Passport

 
  • Completed Claim Form

  • Passport Copy

  • Copy of ID card of Main Card Holder

  • Flight Tickets or e-tickets (departure and arrival)

  • Loss Report;

  • Receipts for charges incurred

 
 

Hijack

 
  • Completed Claim Form

  • Passport Copy

  • Copy of ID card of Main Card Holder

  • Flight Tickets or e-tickets (departure and arrival)

  • Report from authorities concerned showing full details of the event
